Unable to initialize opportunity form

Hi Folks

I have tried clearing the forms cache
re-installing BCM and SQL DE
Uninstalling Quickbooks and Quicken
No disabled items in -about outlook- dialog box
Any other sugestion?

Me again,

I have also tried KB836558 but no change Opportunity form doesn't load
