How do I clear or fix outline numbers

I have a document with outline numbers that will not let me fix them. My
outline looks like:
1.
1.1
1.1
1.1
Even if I try to restart numbering for level 2 all 3 level 2 items change
every time.
I cannot get the outline to number
1.
1.1
1.2
1.3
