How do I add an item to the start menu above Programs?

I have my start menu set to Classic View so it's like Windows XP. How do
I add an item to the start menu, when you click start, and you get the
initial menu:

Expand All Programs, right click on the program that you want to place
on the Start Menu, and then left click on Pin to Start Menu.

That works if I'm in default Vista mode, but I switched to Classic view
CONTROL PANEL -> TASK BAR AND START MENU -> START MENU(tab) -> Classic
Start menu. Pin to Start Menu isn't an option when I do that.

Also, I tried switching to default Vista view, pinning an item, then
switching back to Classic Start menu, and the item was no longer there.
Drat.

But a few programs I've installed have figured out how to put themselves
there. I tried searching the registry for one of the items that is
there, but didn't find the location where it's stored (assuming it's
stored in the registry).
