Switchboard Form

I have created a Switchboard using the Switchboard Manager in MS Access
2000. I have managed to get the table created for the switchboard, however
it hasn't actually created the form.

Can anybody tell me how to get the form automatically created from the
Switchboard Table if at all possible?

Humor me:
What happens if you create a new blank database and run the SBM on that file?
Is a Switchboard form and table created in this new, blank database or just the table?
