Switchboard form does not appear on list of forms

I have created a Switchboard using the Switchboard Manager, but there is no
Switchboard form in the list of forms. Hence, I cannot select the Switchboard
to come up automatically at startup.

It sounds like you may have accidently deleted (or hidden) your switchboard
form. Do you have a table named "Switchboard Items"? If not, then you may
have created your switchboard in another database.

If you do have a Switchboard Items table, then first verify that the
Switchboard form is not hidden. Click on Tools > Options | View tab.
Temporarily place a check in the option to show hidden objects. Dismiss this
dialog. Does your Switchboard form now show up? If not, then the form may
have gotten deleted. Use the instructions shown here to rebuilt the
Switchboard form:

After you create your switchboard you need to set it as the default
switchboard. then it will show up.
