If our product fails the 'verified for windows xp' certification test, are
we given another chance, or we need to pay the full fees again and re-try?

AFAIK, fix for same driver qualifies as "Update hardware and driver test submissions" and costs $250 per OS.
The best is to start from WinXP driver package (which is mandatory).
After you make it passed, try adding other OS packages.
