Outlook 2003 cache mode and Junk mail

Why does Junk Mail filtering in Outlook 2003 only work in Cache mode? This seems like a feature that should work in any mode, especially when connected to an Exchange 2003 server

We are recommending our clients not run in cache mode as a number of problems have come to light, especially working connected to Exchange 2003. Problems with appointments missing, migration issues and profiles when coupled with cache mode mean that it is too much trouble. However junk mail doesn't work.

Because the messages are only evaluated as they are written to the cache.
We are recommending our clients not run in cache mode as a number of

That is the trade-off. For whatever it's worth I've had no significant
trouble with cached mode against either Exchange 2000 or 2003 servers so
far.
