Forms issue

When I place a form on my site it appears on the web as it
should except it has the text, [Frontpage Save Results
Component] displayed at the top of the form. It is very
unsightly. My hosting service DOES have Frontpage
extensions installed.
Any ideas for getting rid of this?

How did you "place" the form in your web site? The only way to correctly
"place" the form page is to publish. Publishing uses the FrontPage server
extensions to correctly configure your form.
