Forms in Outlook

I have created a form with checkbox fields. If someone checks the box and
emails the form back to me, the checkmark disappears. What am I doing wrong?

Well, for a start, you're not telling us what version of Outlook you're using or whether this is a form saved as an .oft file or published to an Outlook folder. Most likely, though, you forgot to click the Edit Read Page button to create a layout for reading the data from the form.
