Can you use form fields in body of Outlook message?

Can anyone tell me how to create a drop-down list, or check box form field in
the message body of an e-mail? I create forms in Word, so thought I might be
able to do the same in email, if I am using Word as my mail editor. It works
at preparation stage, of course, but when the mail reaches the recipient, all
the form stuff has been wiped. What might I be doing wrong? Is there
another option? (Voting buttons isf for my purpose in this case.)

There is no point in using them within a message when you cannot put
programmatic logic behind it (what should happen when they select something?
How are the results going to reach you?).

For voting, you can use the voting options in Outlook when you know the
recipient is also using Outlook.
For multiple questions, it would be better to use a web based solution where
results are directly stored in a database and then send the invitation for
the survey via email.
