Document map doesn't show headings when outline level is greyed

Some of my headings (in a Word template) have their outline level greyed out
in the Paragraph dialog. Even though their level is Level 1 these headings do
not show up in the document map. How can I make these levels active and/or
make them show in the document map?

The outline levels of built-in headings are greyed out in the Paragraph
dialog box, because you cannot change them. Still, you should be able to see
paragraphs formatted with heading styles (or with any style that has an
outline level defined) in the Document Map. Are you sure that you have
applied the correct style to text?
