create address\phone book for visual impaired person

Iam trying to create a address\phone book with large print and a section for
a photo within word, is there a template available?
 
You hardly need a template for this. Create a table and set the font size
large enough for your requirements. If you are using an existing data file
as a source then create a datalog/directory mail merge to place the data in
your table.
