Could not install custom Actions

I am currently running Office 2003 Win XP. Recently I attemped to open a custom form from the Organizational folder and was unable to open it. Warning Error" Could not install custom Actions" was received and Outlook 2003 shut down. I reinstalled the office suite and I am now able to open the form from the organizational folder but i still get the warning error before the form opens. Problems are encountered when forms are sent from (Outlook 2000) to (Outlook 2003) the forms are unabled to be opened
new problem: I was attempting to edit the form code with the Visual Basic editor. When trying to open
Tools-> Macros->VB editor and Tools->Macros->Macros Outlook shut down again and tried to send an error report. I am unsure as to why this happens.

Yours is the fourth case I've encountered of this error message. We don't
know the cause yet, but are trying to find the common thread by getting
answers to various questions:

-- In what version of Outlook was this form created?
-- Does the form contain any custom actions on the (Actions) tab other than
the standard actions?
-- Does the form have any code behind it? (Note that you don't edit form
code with the VBA editor; you might want to repost the VBA crash problem
separately in the microsoft.public.outlook.program_vba group)
-- Can an Outlook 2003 user successfully create a new item using this form?
-- Does clearing the forms cache change the behavior?

I am now able to view the form with outlook 2003, the office suite was reinstalled but the "custom actions" error is still displayed beforte the form appears. However other users in the office are still not able to view the form with 2003

Thanks! The other cases also were forms that were created in Outlook 2000 or
2002. One user succeeded in getting it to work by republishing the form. It
would be interesting to know if you could, in OL2003, run an .oft file
backup of the firm saved with Outlook 2000. If so, then you should be able
to publish with OL 2003.

I have the same problem as well. Basically same details as
HolderTM. I've tried opening the .oft "problem" form
directly in 2003 and get the same behavior...
