Presumably since applying a hotfix or SP, Windows XP now
doesn't know when I have ejected/replaced a CD or DVD from
explorer. How can I return it to its "old" Win2k-like
behavior?

It seems that the problem is something to do with "Autopopulate Volume." The
OS sees the correct volume at system startup but not thereafter. Some
software (nero, IMAPI, etc. may have had a hand in this, or a hotfix).

Correct,

I had the same problem. Formatted the drive reinstalled XP and the problem
is gone.
