What happened to the "other" button in Insert address in Word 2003

In word 2000, there was a button so that you could insert an alternate
address for someone rather than their default address. The window doens't
have a button for "other". Any idea how you can insert an alternate address
from the address book?

It's still there. It's under Addresses on the Contact's individual listing.
Use the dropdown arrow to access Business, Home, Other.
 
I can see the address, but it doesn't automatically insert as it did
previously. The insertion only works for the default address. Yes, I can
see the address, if I click on the individual address and push the arrow,
but I don't really like all of the extra steps, when it used to be done by
just one!!!
