user defined printing with Epson AP5500: still no success

After reading posts, I felt I was on the right track as, I created a form in Server properties of .5" x 3" for slide labels. This doesn't show up in paper sizes though when the printer is selected. Neither does custom in the list.
Doesn't anyone know about this? I have posted in 5 reply follow ups and still no answer.
