Found this in of the forums;

There is a picture of a chair; with the following instructions;

Save the picture below to hard disk any folder, then open the folder and set
the view style to thumbnails
u will see the picture changed!

on following the instructions; I see the thumbnail of a girl instead of the
chair

Can anyone speculate how it is being done?

Would it possible to attach the pic here?

Is the picture format a TIF image file? If so the large image has embedded
in it a preview, or thumbnail, image which is normally a smaller version of
the larger TIF file but in this case the person who created the image
replaced the preview image with the girls' image. There are other image
files that also include a preview or thumbnail embedded so that is probably
how it was done. The thumbnail and other embedded information in images is
also how some virus is spread. Always have a good antivirus scan done on
any image or file you open from any source.

You should not attach any image to a newsgroup message as all messages are
supposed to be in plain text. If you want others to see what you are
talking about you should provide the URL of the image.

There are some "versions" of JPG files that can contain text as well as
multiple images embedded within the file according to quick research I just
did. As for a program or method of creating this, I guess you would have to
research it for yourself as I just don't have the time to do that research.
