Survey Form Doesn't return the form fields

Hello,

I created a custom form based on the Message Class. Now when I run the form
and send it to myself not 1 of the form fields are in the message that I
receive. This is a Survey form and I will be sending it to my external
customers soliciting feedback. I'm using Outllook 2007 without an Exchange
server.

I tried checking "Send form definition with item" and "Use form only for
responses" which didn't help?

If this isn't going to work, what might be a suitable option for emailing
survey forms?

It's completely unsuitable for an external customer survey. The only way to make that form work in your environment is to publish it to every user's Personal Forms library with the "send form definition with item" option turned off.

If you also have Access 2007, a better solution is to use its External Data | Collect Data command to generate HTML-format messages that can be used to collect survey results.
