Custom Survey form problems

Hi I would like to survey people in my organization. Can I send a custom
form definition, rich text, to them and obtain reponses on this form without
the form being published, either on an organizational folder or elsewhere
and without programming? I have tried the sample survey from Sue Mosher's
excellent reference site, but this also does not seem to work on our system.
Using Win2K.

To use the survey form from http://www.slipstick.com/dev/olforms/survey.htm,
you must be able to publish it to the Organizational Forms library on
Exchange or to each user's Personal Forms library.

It is difficult to use a custom form for a survey without publishing the
form, because usually some scripting is required and code does not run on
unpublished forms. One possibility would be to create a message form that
contains controls and fields for the information you want to gather, run it,
and add a To address. Then save the resulting item as a .msg or .oft file
and send it as an attachment to respondents.

Or perhaps you should consider using a web page for the survey.
