Survey Form

I'm part of a volunteer committee and have to send out a survey with 5-6
questions to other associates in the company. I have done VBA a programming
in excel but have no experience with Forms in Outlook. How do i create a
survey form in Outlook and get the response from the associates I send the
form to?

The first step is to ask your Exchange administrator if you can have permission to publish forms to the Organizational Forms library. That needs to be done first, because if the answer is No, then an Outlook custom form is not a viable solution.

If you can publish a form to Org Forms, you might want to use the generic survey form from http://www.outlookcode.com/d/forms/survey.htm
