Strange windows activity, not a virus

Hello all,

I run a network of about 150 PC's and one PC is suffering from a
problem I have never seen in my life. When the user is just working in
any program (generally she is in Office/Outlook for work) all of a
sudden windows will pop up and the PC hangs. Now for the strange part!
The windows aren't ads or pop-ups they are windows components and they
arrange themselves into four equal squares filling the screen. Like
below:

The last time it did this the four windows were: Outlook, Excel, Word,
and the run command box was up. Outlook and word had been running but
it opened Excel and the run command on its own. The PC was frozen, but
I could use taskmgr and close the run process and the PC was usable
again. It happens to her multiple times a day and no matter what
program she is in.

I have scanned the PC about 10 times with every scanner available and
it shows clean each time. What could be causing this?

What do you mean by "windows components" ?
What do you see in the event logs?
Is this system fully patched?
What scanners did you run? Malware/spyware as well as antivirus, I trust.
