Simple e-mail survey -- help!

How do I conduct a simple survey via e-mail using a custom form with response
buttons? I have looked but can't seem to find simple 'how-to' answers. I
just want to send an e-mail to a distribution list that asks a question and
gives them 'yes' and 'no' response buttons to choose from. When they click
one of the buttons it let's me know their response. Please help.

There is no simple answer to find. The available approaches depend on a variety of factors, the most important of which is whether the recipients are using Outlook.
