Need to conducts a simple e-mail survey - please help!

How do I conduct a simple survey via e-mail using a custom form with response
buttons? I have looked but can't seem to find simple 'how-to' answers. I
just want to send an e-mail to a distribution list that asks a question and
gives them 'yes' and 'no' response buttons to choose from. When they click
one of the buttons it let's me know their response. Please help.

If you're using Outlook (not Outlook Express) you can do it by composing
your message and clicking "Options" before you send where you enable "Voting
Buttons."
