Need a simple email way to do a survey with employee feedback

I need a simple way to do a 3 question survey with employee feedback on each
of the questions. It must also have tracking of some sort. Any ideas?
 
You could use a web based solution (there are many) and send out the survey
requests via email.
If you want do to this all mail based and you want to organize your results
