Creating a simple interactive Survey

Is there an easy way to create an interactive survey with multiple questions
each with several predetermine answeres that recipinets pick from, or a
fill-in the blank field after questions in an e-mail message?

I'm not Visual Basic literate, but I suspect that this could be done easily
with VB, but is there any other way?

Unless you're working in an Exchange environment where you can publish a form to the Organizational Forms library or are using Office 2007 Professional, Outlook mail messages are not an appropriate solution for this task. Instead, consider using Microsoft InfoPath or a web page survey.
