Separate Custom Form For Meeting Requests And Appointments

I created a custom form, published it to the Organizational Forms Library,
and imported it into Outlook 2007 on a test machine. I basically followed
these steps:
http://www.petri.co.il/customizing_new_meeting_request_outlook_form.htm

It works perfectly with one exception. The form load whether I select a New
Meeting Request or New Appointment. Is there a way to prevent New
Appointment from using the custom form while allowing New Meeting to use the
custom form? Since they are both the same message class, IPM.Appointment, my
gut tells me this can't be done.

Hopefully I am incorrect and some will point me in the right direction.

Thanks!

There's the problem. I did start with the MeetingRequest for which does have
a MessageClass of IPM.Schedule.Meeting.Request but when I try to publish it
to the Org Forms Library, it changes the message class to IPM.Appointment.

Is there something I am doing wrong or is there a work around?
