Another possibility is that you dutifully used the "Safely Remove
Hardware" control before removing a card, and you haven't re-booted
since.
This control will assume that you wanted to "unplug" the (internal)
USB connection it uses, and will therefore deactivate the reader until
the system is rebooted (or until you open up your machine, unplug the
USB cable to the reader, and then plug it in again ;-).
I no longer the "Safely Remove" feature--I just wait a respectable
amount of time since my last write access to the card, and remove
it. (I also disable write cachine in the card reader's properties,
under "policies" to make "Safely Remove..." unnecessary.)
