Saved document but changes aren't there

I'm using Word 2003

I made changes to a document using the save icon on the menu bar. It never
gives an error message or a prompt to save upon exiting, but when I open the
document back up the changes aren’t there. This has happen on more then one
occasion with the same document. Has anybody else had or is having this
problem, if so is there a solution for it? Thanks.

If the document was opened directly from an email attachment, it was saved
only in a temp folder and deleted when you quit Word.

No it's a document that's off the network in a shared drive. Which is
sometime hard to track down who did what to it. But everybody is claiming
that they saved it and no two person are in at the same time. First I
thought someone have saved another file, but the are saying no.
