Save Results Form to Email prompts for User Name and Password

My form feedback to email (save results) does not work some visitors.
Instead, it prompts the visitor to enter a user name and password? The weird
thing is that it works for some computers. I have Frontpage 2003 and the
server is Go Daddy. Go Daddy has not been very helpful.

In my opinion the problem is a permissions problem on the server which
should be fixed by GoDaddy. However, there is also a problem in the form
properties:

"B-Email-Subject-From-Field="TRUE"" implies that a form field should be
used to get the email subject. However, there is no field defined.
Also, S-Email-Subject="Contact through Website" implies that the email
subject is fixed and not collected from a form field.
