Rookie Question Outlook Form

I am new to forms in outlook.
How can I create or where can I find Information about how to create a
form which creates an appointment, a task and an email in one step?

I need it for a small hospital an outlook should manage the patients

e.g.

The patient should come on the 1st of October an will stay for seven days.

So i want the form to do the following:
An appointment from 1.10.2006 to 8.10.2006
A task on 1.10.2006
Send an Email withs some data from the form on to some recipients.

Is it possible to "mix" different types like Tasks and appointments in
one form?

No, each individual form must be based on one of the standard form types.
How can I create or where can I find Information about how to create a
form which creates an appointment, a task and an email in one step?

Create each of those as a separate item. The details depend on where you want to store them and whether they will also use custm forms.
