Removing a custom form

Hi,
I created a custom form and stored it in the Personal Forms Library. I'd
like to delete the form but don't see a way to do this. Any help would be
appreciated.

Use the Forms Manager to Set the library, then click the Delete button for
the selected custom form. The Forms Manager is under Tool -> Options ->
Other -> Advanced Options -> Custom Forms -> Manage Forms.
