Places bar missing in open dialog

For some reason, the "places bar" (containing a link to "my documents" among
other useful places) is missing in open dialog boxes on my Windows 2000
system. It shows up sometimes, but I can't make out any pattern about when
it will or won't show up (mostly it's missing).
 
Are the places blank (likely) or the places bar has been totally removed
(unlikely)?

The bar is actually completely missing, not blank or empty. Just gone. It's
like using Windows 3.0.
