Password Protection

How would I go about setting up a password protection for
a web page???

What I want to do is to develop a form for users to
submit information to me. When they click on the link to
the form or when they try to access it from their
favorites, or when they try to type in the direct URL,
the form will not display until they enter a password.
It can be the same password for all users, but something
that I would change a couple of times during the year.

I want them to enter the password everytime they access
this particular page...

Any ideas?
