Outlook form - Form does not display when i hit SEND

the only part that displays is the top part (from sent, to &subject); i've
saved form & attachments it to my personal forms library & published it
there. What else can i check? Thanks.

I had this same problem when I first started using forms and realized that
your "Edit Read Page" needs to be identical to you "Edit Compose Page". You
can just cut and paste all the information to your Edit Read Page. I hope
this helps.

HOLY COW!!!!! you are a genius!! i've spent 2 weeks reading everyone's
questions on this site; reading another site & i must have missed this little
jewel of information. i had created another form & it had been working
fine--thank you, thank you, thank you!!
