OT: Safesurf Rating

I've tried repeatedly over the last few months to use Safesurf's rating form
and every time I try I get an error. I've written Safesurf about it with no
response.

Has any one been successful with their form lately?

Anyone have any helpful info?

Have consider using the ICRA instead, which does the same thing?
