Losing Monitor Refresh Rate

My computer (XP, P4, Radeon) keeps losing its refresh rate & dropping down
to a painfully low rate. When I go into settings to change it, it shows that
it's still set to 100 mhz, but it isn't. I have to change it, and then
change it back.

It also keeps forgetting what kind of monitor I have, reverting to "default
monitor."

This may not be the right place to get an answer, but I'm not even sure --
is the problem OS based? BIOS based? Is it my video card? Video drivers?

ATI doesn't bother to answer support requests, so any assistance is
appreciated.
