layout columns

I was trying to do a 3 column affair. I wanted a title at the top with the 3
columns with numbered lines below. I couldn't figure out how to do this.
Trying everything I messed up the column layout .... so now when I bring that
up it shows the a very complex grid and it will have the 3 column layout but
this grid... and I can't seem to get rid of it.
What I need is to first get rid of this and then a simple solution to making
this form. I have done things like this before in other word types and it
was easy... this is driving me crazy.
