Is there a common form that anyone can respond with their machine and send it by their mail?

Hello:

I have been creating many forms during past years. These forms usually for
use of visitors input for my pages(photography gallery etc.). But suddenly
I got problem: People who's use their computers in the office can not send
me their email form. Is this happens because of outlooks difference on it's
versions?

Is there a common form that anyone can use it on their machine... (I
mean,what ever email program they use) and they can send it by email? If
there is such a thing, where can find the html code of it?

Thanks for reading my question and I hope some one will help me on this
subject.

The only type of message that you can guarantee will work with all email clients is a plain-text message. You can always provide a link to a web page form on your site.
