Windows XP Internet Explorer shortcut

Hi everyone, could really do with this fixing because its really starting to bug me now. Basically what is happening is when i double click on the Internet explorer icon, it creates a shortcut instead. Also I've tried to open IE through the start menu and on the toolbar, but when i do that a window pops up saying to i want to send error report to Windows or not.

Now I've tried changing the default action of the double click, and run updates and tried to install a newer version of IE to see if that made any difference.

Really would appreciate any help or suggestions!!
