Internet doesn't work-loging is slower

I can't log at the speed i did the other day.
My internet doesn't connect me with the phone line(Att
Global).
It takes 5 minutes for Windows to totally start up.
My McAfee Virus scan sent me a notice when i went into
User Accounts under Control Pannel. it said when i clicked
on "change the way users login to the accounts" or
something like that, that that file had a harmful
attachemtn to it. the attachment was called
nursmgr.cpl/nursmgr.hta

i didn't decide to continue on and my computer get me off
the user account sceen.

do i havea virus?
do i need a windows update?
please help and thanks.
 
Do you know why you have the attachment?
Are you a nurse?
Can you go in and delete the attachment?
 
im not a nurse- can i get some real help- the file is in
system32 folder in the windows folder. i have the
attachment because windows put it there. if i delete the
file will windows not work?
