What are the criteria for what appears in the address bar
listing of previously visited sites?
Some sites appear in the history but never make it to the
address bar list.
Some sites have been entered as Favorites, appear in the
address list, and are still there when they have been
deleted from the favorites as well as from the history.
Why do some sites make it to the address list, but others
do not? Is there a rule?
Any info will be appreciated.

Did you check your computer for any spyware? Do you have antivirus
software installed? Do you have a firewall running?

If you don't have any anti-virus/firewall software installed then you
could do an online scan at RAV:

If you run Windows XP and you don't have a firewall installed then at
least turn on XPs firewall:
