I want to do a multiple fillin box for when a template is opened

Hi

I am trying to do a template where when you open it, you have a fillin box
that opens up, that has multiple entries. Instead of a saying in one box
Letter to in one box then another box with Salutation etc. I want these to be
on just one box. I have one for a fax left by a previous employee, but I just
can't figure out how she did it.
