How do I set up a "Custom Action" in Outlook?

I am trying to set up a custom action in Outlook 2003, but am having no luck.
Searching "custom action" within help did not supply any answers.

Custom actions are 3rd party utilities, often written in C++. See
http://www.slipstick.com/addins/custom.htm for a list of actions available.
