How do I restore a deleted switchboard form?

I created a switchboard by accidently deleted the switchboard form... is
there any way to create a new switchboard form? Is there any way to be
promted again by access 2003 if i want to creat a switchboard?

If you deleted it, it's gone.
I assume it is one created using the built-in Switchboard Manager.
If you still have a table named "Switchboard Items", rename the table
to "SwitchboardOld".

Then remake the switchboard using the Switchboard Manager.
Tools + Database Utilities + Switchboard Manager

Add something to the new switchboard.
Save it.
Then go back to the tables and delete the new Switchboard Items table.
Rename "Switchboard Old" to "Switchboard Items".
That should be all you need do.

If it wasn't the switchboard created by the Manager, simple make a new
one using an unbound form.
