How do I get a sound and a desktop alert for every new message?

Hello,

I would like to get a sound and a desktop alert for each new message. I am
only getting a sound and an alert some of the time. I have it set to play a
sound and give an alert. What is going on and how do I change it? I tried
looking at the rules and alerts (when I finally found it) and got nowhere.
It was not at all clear. Thanks for any help.
