Help! Urgent. New Custom Form cannot be displayed

I spent hours working on a new contact form today. I published several times
along the way to save changes, under 3 different names, to protect any
changes before making permanent. Form is called LT-Lead2, 3, & 4.

Now outlook says 2 & 3 can't be displayed. 4 opens, however doesn't not
incorporate most recent changes. I've used the:
"C:\Program Files\Microsoft Office\OFFICE11\OUTLOOK.EXE" /c
"IPM.Contact.LT-Lead2"
and only form 4 still opens. Please help!!!
 
Try clearing your forms cache. Tools, Options, Other. Advanced Options,
Custom Forms, Manage Forms, Clear Cache.
