Header and chapter names?

Is there a way to have the header contain the title of the work,
company name, my name and a variable chapter name that varies as do

You should be able to just type the title, company name, and your name
into the header because they are static text. If the chapter name
appears in the text and is formatted with a certain style, then you can
insert a { STYLEREF } field in the header to pick up that information.

Yes. Choose "Different odd and even" headers and footers in File | Page
Setup | Layout tab. Then type the title into the even-page header, and
insert a { STYLEREF } field into the odd-page header.
