Forms and EMail

I am trying to set up a short form on the web which will allow users to
email the results to me once the submit button is pressed
When i use the page I get an error message as follows:
FrontPage Error.
User: please report details to this site's webmaster.


Webmaster: please see the server's application event log for more details.

What am i doing wrong?

The server **must** have FrontPage extensions installed, and then you **must** publish using
http://yourwebsitename .net or .com or .org ( whichever is appropriate ) as the publish to address

For Front Page Forms to function, your site must be
hosted on a server that supports the Front Page
Extensions (to process the form data). Additionally, your
account must be configured to enable email transport.

The "webmaster" referred to in the error message you are
receiving refers to your host. Contact them.
