Form Question

I have a form that has 2 pages. The second page has different layout from the
first page. Is it possible to force the second page layout when a new page is
added.

What kind of a form is this that will require additional pages to be added?
Depending on what exactly you want to achieve, add a section break after the
first page and format the two pages as you require them. Section 2
formatting will be retained over additional page breaks. But if you want
form fields to be ready placed on page three, we would need a lot more
information about the way the form is laid out and under what circumstances
the third page is added (and how).

And if it's just a letterhead, see

discourage the use of form fields in a letterhead (or making a letter
template a protected form).
