form not functional, server side seems fine

I spoke with hosting and FP SE are installed properly, and they checked
"their side" but I'm unable to get this form to work. Not only does it not
send anything to the file (I'm wanting form contents sent to folder in
_private), but the validation doesn't work, and it doesn't send you to the
confirmation page I created.

I've not done a form before, so I'm assuming I've done something in error.
I would appreciate feedback. Thanks.

Most hosts only allow the email to work if it's email address within the
same domain...yours isn't.

The host also has to allow the fp form handler to work...and permissions
have to be set correctly on _private (read/write I believe)

The forms name should be changed - form and field names must not contain
spaces.

The page must be published to the server using File->Publish in
FrontPage, and using HTTP method.
The destination address should be http://www.theitaliangreyhound.net

Or, open the remote site in FrontPage and run Tools->Recalculate
Hyperlinks
This should correct action="-- Webbot Self --" to the correct code.

thank you both for the responses. I made note of the email issue, but
discovered that I'm on a shared server, so anonymous uploading is Not allowed
(I just called my hosting co)

in lieu of uploading files to my server, is it possible to use the FP form
to email form responses AND attachments from the form? I don't see this
covered in my FP Bible or FP for Dummies (or I've missed it, which, dare I
say,is possible).
